It is the Training Program’s goal to deliver the highest quality training to Child Welfare staff across the state. As a Trainer, you play a critical role in the quality assurance process. During the first year of implementation of a new curriculum, the Training Program will consider the curriculum to be in a field test status. During the field test period, the Training Program is asking Trainers to provide us with feedback regarding the delivery of the curriculum. After you train the curriculum, please complete this form and hit the submit button.

After one year of implementation, a summary of the feedback received from Trainers will be compiled. The information will be used in deciding the future direction of the curriculum. We appreciate your assistance in giving us information regarding the delivery of the curriculum.
